The problem
Medicare, Medicaid, SNAP, STAR+PLUS — all funded, all available. But for millions of multilingual families, they’re invisible. Not because the programs failed. Because the navigation did.
Discharge instructions in English. A parent who speaks only Hindi. A home health authorization delayed nine days while a family struggles with a system in the wrong language.
Medicare. Medicaid. SNAP. STAR+PLUS. Each requires a separate application, a separate agency — in English. Families who qualify leave without benefits they’ve already earned.
When families can’t navigate care in their language, the ER becomes the default. At $8,000–$12,000 per avoidable visit, this is a $47 billion annual system failure.

His mother Rosa speaks only Spanish. When her prescription was denied, the letter arrived in English. He never discovered the program that would have covered her supplies at no cost — for two years.
His parents arrived from Guangzhou in 2023. His mother has early-stage dementia. Wei doesn’t know they qualify for Medicaid. Every resource exists. None reached him.
